The city document intended to tell disabled individuals how they could possibly obtain tax breaks on special insurance.” “Don’t kill me, I’m still alive” was the first reaction of a letter recipient to City Hall. “Why are you guys sending me a document telling me I’m dead.” Shocked city staffers say they somehow missed the line expressing regrets about the individuals dying. The chain of errors apparently began after one family called to tell the city who received the insurance tax break letter the family member was, in fact, dead and didn’t need the insurance. The regrets to that family somehow wound up in the document going out to hundreds of living elderly people eligible for the special nursing care insurance. “We want to do our job completely, and make no more mistakes,” the City Office says. “Sorry for the bad feelings.” The City Office has sent letters to each of the 334 individuals who were stunned to find they “had died” and apologized to them. |
