Hospital Releases 5 Year Mishap Report

Okinawa Prefecture Naha Hospital has released a report covering mishaps from 1995 to 2000, including nine cases of accidental death. The 561 incidents cited in the report include one instance of a patient falling out of bed and dying. On another occasion a patient died because the apparatus dispensing an intravenous drip malfunctioned and delivered too much of the solution.

The hospital insists the 561 incidents are not a list of medical mistakes. It says it is trying to improve its medical procedures by making the report open to the public.
